ATTENTION UNIVERISTY COMMITTEE CHAIRS: Please provide the Office of Academic Affairs (Unit 8358),
copies of all meeting minutes for 2005-2006 and an annual committee report, due by May 12, 2006, that
provides a very brief summary of the year's work to include: an evaluation of particular members (who
functioned well/poorly); tasks still in progress; suggestions for next year's chair; a description of the size and
makeup of the committee; a recommendation as to whether the committee is necessary; and suggestions to
make the committee more efficient
