"Upon advice by the University Physician or the Director of Student
Health Service that a student should be confined to rest in the residence hall or local off-campus address or at the home of record for
a short recuperation, or that a student should leave the University
for home for an extended period of convalescence, the Dean of Student
Affairs shall so notify the instructors of the classes in which the
student is enrolled. In the latter instance a decision on withdrawal
from the University for medical reasons then shall be made by the
student in consultation with appropriate academic and counselor advisers."

All faculty advisers are reminded that if a student wishes to declare
or change their minor field of study, the procedure is the same as
that for declaring or changing their major -- to go to their Academic
Dean's office to initiate the declaration or change.

claims for January 1 through September 30 as soon as possible. DO
